Licensed Clinical Social Worker (LCSW)

We are actively recruiting a Licensed Clinical Social Worker (LCSW) to provide culturally sensitive mental health services to the communities by providing assessments, referrals and follow-up appointments includes developing plans with the client and implementing therapeutic treatment plans in strength-based model in both the doctors’ offices and the individual counseling office.

 

Brief Description of Responsibilities: This position will be responsible for the following:

  • Provides direct behavioral health and medical social work services to individuals, families, groups and communities, under the supervision of the Medical Director.
  • Provides specialized counseling groups or classes for adjustment to illness, adjustment to pain, parenting classes, depression, grief, anger management, teen issues or on other topics within the scope of license and training.
  • Supplies case files including assessments, treatment plans and progress notes and other reporting and statistical data in a timely manner as required by the law and clinic guidelines.
  • Discusses progress toward goals with client such as controlling weight, controlling pain, adjusting to stress, or substance abuse education and support services such as observing urine tests.
  • Refers clients to supportive community services and in-house medical services or support programs to supplement office treatment and counseling.
  • Periodically provides community mental health presentations and power point presentations on pain management, childhood mental health issues, or stress management workshops to the community or clinic staff.

Work Experience and Education Requirements: Licensed in California as an LCSW with two years (3-5 years preferred) of clinical experience post masters degree and preferably some experience in medical social work. Current Basic Cardiac Life Support (BCLS) certification for infants and adults. Community Clinic setting experience, preferred. Must be familiar with providing specialized counseling groups or classes for adjustment to illness, adjustment to pain, parenting classes, depression, grief, anger management, teen issues or on other topics within the scope of license and training. Comprehensive knowledge of community resources, and public and private benefits programs. Familiar with various funding sources such as Medi-Cal, Medi-Care, etc. Bilingual (English/Spanish) with comprehension of medical terms in both Spanish and English preferred. Must have a valid California driver’s license and acceptable driving record.

Eligibility Access Worker

We are actively recruiting an Eligibility Access Specialist to provide eligibility and intake support services to ensure continued health care coverage and health access to children and adults.


Brief Description of Responsibilities: The successful candidate will be responsible for:

  • Determining patient eligibility for appropriate government sponsored programs.
  • Verifying eligibility for medical insurance benefits for all clinic patients.
  • Following up on all pending applications to ensure enrollment process is accurate and complete.
  • Maintaining a policies and procedures manual that includes a user’s guide for referrals, including physicians in all specialty areas for children and adults.
  • Assisting directors and staff to collect data, track outcomes, and support strategic planning processes.
  • Assisting with developing, editing, producing and disseminating products and reports including: policies and procedures, public outreach materials, project reports and presentations and other project information and communication.
  • Maintaining the privacy and security of protected health information, the confidentiality of all information and conducts all aspects of patient care in an ethical and professional manner in accordance with federal, state rules and regulations.

Work Experience and Education Requirements:

  • Must have knowledge of eligibility requirements and application process of all government sponsored health insurance programs.
  • One year of directly related experience required, experience in medical/healthcare environment preferred.
  • High School diploma or equivalent, Associate’s Degree or certificate in a related field preferred.
  • Demonstrated “detail approach” to all administrative and organizational duties.
  • Excellent verbal and written communication skills. Must be bilingual in English and Spanish
  • Ability to take initiative to identify and propose new tasks and procedures.
  • Ability to foster a safe environment where people will feel comfortable to share private pertinent information for effective treatment.
  • Ability to travel to work sites as required. May be required to provide eligibility access support within the clinic as well as assisting our mobile outreach clinics as needed.
  • Demonstrated self-motivation, confidentiality, and flexibility within all areas of responsibility.
